About Chronic Kidney Disease

Chronic Kidney Disease, also known as Chronic Kidney Failure, is a condition in which the kidneys no longer work as well as they should. When this happens, the kidneys are not able to perform their job of filtering the blood and clearing toxins with the same effectiveness. The most common causes of chronic kidney disease are uncontrolled blood pressure and diabetes. Whatever the cause is, it is crucial to intervene early to be able to slow the progression and reduce the risk for further complications.

When CKD is mild, most of the time there are no symptoms and you may not notice anything wrong at all, but if it progresses over time to a more severe state, it can lead to electrolyte problems, build up of fluids and toxins and uncontrolled blood pressure. Eventually if it is severe enough dialysis and kidney transplant may be considered

With proactive treatment to manage underlying conditions and avoiding things that can accelerate the progression of your kidney disease and optimize renal function, we can help you and your kidneys stay as healthy as possible, for as long as possible.

The Ibis Health Program

As part of your Chronic Kidney Disease Care Plan, Ibis will ask you questions about your appetite, fluid intake and urination patterns, any swelling, fatigue or other symptoms. Ibis may also collect data such as blood pressure, heart rate, oxygen saturation, blood sugar levels and weight. When this information is combined with information from your PCP and specialist teams, it presents a regularly updated summary of how your kidneys are doing. At the same time, Ibis will offer helpful tips including medication reminders, lifestyle goals, and other information, and you can follow your progress on the Ibis platform, gaining confidence as you watch your data improve with your effort.

How can I regain control?

The treatment of Chronic Kidney Disease first involves targeting the treatment of any underlying conditions such as high blood pressure or diabetes. In addition, ensuring that you avoid medications that can damage the kidneys and making sure you intake appropriate amounts of fluids, you can often times help arrest the progression of the kidney disease from mild and inconsequential to more severe. Also coordination of care with your PCP and specialists helps monitor your kidney function so that it can be intervened upon before it progresses.

Additionally, lifestyle changes such as diet and exercise are required. All of this can be overwhelming, but Ibis Health can help. We work with you to create an individualized self-care plan for your personal needs. With daily guided steps we work together to improve your overall health so you can gain the skills you need and confidence to manage your kidney disease.
