Many Medicare beneficiaries aren’t aware that they may be eligible and covered for ongoing, personalized health support that could ease their daily challenges. If you are living with two or more long-term chronic conditions, you may already qualify for Medicare-covered services called Chronic Care Management (CCM).

This guide will help you understand what CCM is, how to know if you qualify, and what enrolling with Ibis Health actually looks like.

How Can Chronic Care Management Support My Health?

Chronic Care Management, or CCM, is a service covered under Medicare Part B. It supports beneficiaries experiencing two or more chronic health conditions. Unlike traditional medical visits, CCM provides continued guidance between appointments, helping you stay on top of symptoms, medications, with a personalized care plan from the comfort of your home.

The Ibis Health program includes regular check-ins with your Ibis care team, remote monitoring using tools like the IbisHub tablet, and shared communication with your primary care provider and medical specialists.  It is not designed to replace your current healthcare providers but to support your existing care with added support, consistency and structure.

Do You Meet the Criteria for CCM?

To be eligible for CCM, there are a few important factors that Medicare considers. You must be enrolled in Medicare Part B. You should have at least two chronic health conditions that are expected to last a year or longer. These conditions should require consistent attention, such as medication management, lifestyle adjustments, and symptom monitoring.

A commitment to actively participate in a CCM program will be vital to the success of your health journey. If you are unsure whether your conditions qualify, or if your situation meets these standards, Ibis Health can help you verify everything before engaging with the Ibis program.

What Chronic Conditions Are Covered?

There is a common misconception that only serious or rare conditions are eligible. Many of the most common health concerns among adults and older adults qualify.

For example, if you are managing high blood pressure and diabetes, or arthritis alongside asthma, these combinations typically meet the criteria. Other common qualifying conditions include high cholesterol, chronic heart failure, chronic kidney disease and memory-related conditions. Individuals undergoing long-term treatment for cancer or living with HIV may also be eligible.

It is less about how rare your condition is and more about how consistently it affects your daily life. If two or more conditions require regular attention and could worsen without active support, you likely qualify for CCM coverage.

What Does Medicare Cover Through CCM?

CCM covers a wide range of services aimed at keeping your health stable and your care coordinated. This includes regular communication with your Ibis care team, ongoing updates to your care plan, medication reminders, and symptom tracking using a digital tablet and vitals monitoring devices aligned with your chronic conditions.

With Ibis Health, patients also receive the IbisHub, a digital tablet that simplifies remote symptom monitoring and keeps everything connected to your care plan. The Ibis Health team shares relevant updates with your primary care provider, so they are always informed about your progress.

What Does It Cost to Enroll in CCM?

If you have a Medicare supplemental policy or Medicaid, Chronic Care Management is usually fully covered. There is often no cost for qualifying patients.

If you do not have supplemental coverage, there may be a small monthly copay. Most patients find this a manageable cost considering the scope of services provided. It is important to understand that this is not an upgrade or a new policy. It is part of what you already have access to under Medicare Part B.

What Does Being an Ibis Health Member Look Like?

Engaging in CCM through Ibis Health is designed to be simple and stress-free. Your Ibis Health journey begins once your Medicare and chronic conditions are reviewed for eligibility. Once confirmed, and you consent to participating, you will be enrolled.

An IbisHub tablet and appropriate condition specific monitoring devices will be shipped to you, and a remote video session with an Ibis clinician will be scheduled. The next call will be with your dedicated Member Advocate, who will check in with you regularly and serve as your primary contact for CCM support. From there, the care process begins. You benefit from real-time support and your primary care and specialty providers remain involved with updates based on your experience and progress.
