The Ibis Health Program is for Medicare Part B eligible persons who have 2 or more chronic conditions and may have one or more of the following challenges:
- Manage multiple medications
- Have multiple medical providers
- Experience symptoms often
- Irregular vital monitoring data
- Live alone or needs extra support from caregivers

Ibis Health Services Provides
Routine Telehealth Visits
Check in regularly with your dedicated Ibis Nurse Practitioner or Physician virtually through your IbisHub™ tablet. Your Ibis Care Team is always available by phone.
Timely Monitoring Support
When something isn’t quite right with your vitals, your dedicated Ibis Nurse will promptly reach out to you and provide guidance for successful recovery at home.
Daily Self Care Guidance
Receive reminders every day through your IbisHub™ tablet for medications, vital monitoring, exercise, appointments, healthy nutrition, relaxation, and more.
Healthy Living Choices
Learn to live healthy at home, stay independent, and avoid unwanted trips to the hospital. Your dedicated Member Advocate provides individualized education for your health needs.
How Ibis Works For You
Ibis services are covered
by Medicare Part B
If you have 2 or more chronic conditions, Medicare Part B will reimburse Ibis to help you to manage those conditions.

You set your own goals with the Ibis clinical team. You review your progress regularly with your care team and set new goals.
Monitor
Your Health
Recording your daily vitals is easier than you think. Ibis will provide you all medical devices and teach you how to use them.
Stay
Connected
Your IbisHub™ tablet is always connected and clinicians monitor your vitals daily. You can also send messages directly to us anytime.
See Your
Own Data
You can access your health data through IbisHub™. Monthly reports are mailed to you and your doctor to track your progress.
